Medical case management is a range of client-centered services that link clients with medical care, psychosocial, and other supportive services. This service ensures timely and coordinated access to medically appropriate levels of service and continuity of care, through ongoing assessment of the client’s strengths and needs.

Our case managers hold the minimum of a Bachelor’s Degree, possess a working knowledge of HIV and receive extensive ongoing training through the Pennsylvania Case Management Coordination Project of the PA Department of Health. All CCWS case managers are supervised by an AIDS Certified Registered Nurse (ACRN).
